WE ARE GLADLY ACCEPTING NEW PATIENTS. New patients may apply by calling or by downloading and completing the New Patient Form below. New patients at JFMC are asked to complete several important forms so that we have the information needed to give you the best medical care. At your first appointment you will be asked to complete a medical history form and the receptionist will photocopy your insurance cards. HIPAA privacy forms will also be completed, per new federal law.
